Certificate of Completion Reprint Request
If you're in need of a reprint for certificates of completion fill out the following request form. In the message are please the program completed and year of completion. Due to the privacy of our graduates we will require additional identification prior to issuing reprints.

CERTIFICATE VERIFICATION
In order for us best serve this request submit the following request and attach any documentation, such certificate of the individual and forms in need of filling out. The form is to be filled out with the requestors information then, in the message section state the individual on the certificate followed by attachments. Thank you in advance!
